If you have received your PFA or PoleMoves Certification, submit this form to receive access to the PFA Certified Instructor membership. Fill in as much information as you can to help us find you. If you are already showing up on the PFA Certified list with the correct name, level and certification date, you do not need your PFA number or a photo of your certificate. Please allow up to 72 hours for verification. Once you have been verified, log in, go to "My Membership" and click on the "View" button underneath Certified Non-Member:
